The MaxAccountability.org team prepares formal letters and submissions calling on government agencies, elected officials, regulators, and private sector leaders to take action and implement reform.
Each communication reflects our commitment to truth, transparency, and change. We write to hold institutions and individuals accountable, to call out systemic failures, to inform, and to demand legislative and operational change – particularly in cases involving unsafe workplaces, suspected phoenix activity, and failures in duty of care.
In the interest of transparency, we will publish all formal responses received. Where no reply is provided within a reasonable time frame, this will also be noted. Our goal is to ensure that silence and inaction are visible to the public – just as clearly as the calls for accountability.
These letters are more than words on a page – they are part of a growing record of advocacy, and public accountability in honour of all injured and killed workers who deserve protection, justice, and respect.

We believe that transparency matters – not only in what we say, but in how others respond.
Silence is also a form of accountability, and we will document both action and inaction here for public awareness.
